Under the direction of the RN/MSW Case Manager, the Discharge Planner assists in the coordination and implementation of discharge plans, in accordance with existing federal, state, and local standards. As directed by the RN/MSW Case Manager, the Discharge Planner/Case Management Assistant coordinates post-discharge services as identified by the interdisciplinary team and ordered by a physician. The Discharge Planner/Case Management assistant is responsible for documenting per Steward Healthcare policy. The Case Management Assistant communicates and collaborates with the interdisciplinary team through verbal and written communication. Adheres to confidentiality policies specific to communication, record keeping, and coordination of services.

  1. Education: High school graduate or equivalent.
  2. Experience: 1–3 years of acute care hospital experience/or related experience.
  3. Certification/Licensure: BLS certification required.
  4. Software/Hardware: Strong knowledge of Microsoft Office applications – Word, Excel, Access, PowerPoint.
  5. Other:
  6. Understanding of the health care delivery setting.
  7. Excellent computer skills, knowledge of Microsoft programs, data analysis.
  8. Maintains attention to detail in a fast-paced work environment.
  9. Maintains a pleasant, tactful manner when working with people in stressful situations.


  1. Healthcare experience/active enrollment in a healthcare related degree program is preferred.
  2. Knowledge of community resources is preferred.
