JOB SUMMARY:

The following statements are intended to describe the major elements and requirements of the position and should not be taken as an all-inclusive list of responsibilities, duties, and skills required of individuals assigned to this job.

Provides direct and indirect use of critical thinking skills and problem solving abilities in coordinating patient transfers. The transfer center RN will place external patient referrals for direct admissions to various levels of care within facility under the direction of physician acceptance. Able to responsibly communicate with physicians, directors, charge nurses and co-workers effectively for efficient patient throughput within the transfer center. To participate in quality and process improvement in the department during all phases from planning, implementing, and sustaining efforts. Duties assigned per Director of Nursing accordingly for continued maintenance and growth of transfer center.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

Consistently supports the hospital’s Spirit of Serving Standards, in line with the Mission, Vision and Values of St. Joseph Medical Center.

Follows the guidelines related to the Health Insurance Portability and Accountability Act (HIPAA), designed to prevent or detect unauthorized disclosure of Protected Health Information (PHI).

Promotes a culture of safety for patients and Associates through proper identification, proper reporting, documentation and prevention of medical errors in a non-punitive environment.

Maintains staffing within productivity standards and investigates variances and reports findings in a timely fashion. Adjusts staffing for acuity and unusual situations.

Monitors and trends Performance Improvement activities and makes suggestions for improvements as needed. Appropriately involves the nursing staff in this process.

Strives to improve Employee Satisfaction

Is responsible for ensuring that the Patient Advocacy process is adhered to.

Has an excellent working knowledge of patient rights and other regulatory standards.

Ensures nursing staff is educated about policy changes and performance expectations.

Participates on committees outside the department and represents the department in a professional manner.

Interviews, hires, and supervise staff members. Maintain, track and trend the productivity of employees.

Prepares payroll and complete monthly work schedules in accordance with budget and to ensure adequate staffing levels and minimal overtime. Reports labor variances as required.

Performs employee evaluations in a timely manner; utilizing standard trended individual and department performance measures. Ensure competencies are met and competency checklists are appropriately maintained.

Working knowledge of the capital and budgetary process.

Supports, reviews, and understands The Joint Commission’s compliance activities and expectations.Ensures departmental compliance with regulatory standards.

Actively engaged in cost control with regard to quality outcomes without compromise to patient safety or quality of care.

Supportive of the compliance program set forth by SJMC and demonstrated by:

  1. Enforces all compliance policies as they pertain to his/her area.
  2. Adheres to dealing appropriately and fairly with employee misconduct.
  3. Upholds the Code of Ethics and Corporate Compliance.
  4. Provides and assures timely compliance education as requested by the Compliance Officer and/or through corporate initiatives.
    Sets an example to all staff in their daily activities.

MINIMUM KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:
WORK EXPERIENCE:

Current Texas Registered Nurse Licensure; BSN preferred.

Current BLS Certification

Current ACLS Certification

Ability to read and communicate effectively in English, both verbally and in writing.

Additional Languages preferred.

Basic Computer skills
